PLEASE LABEL EPPENDORF TUBES IN THE FOLLOWING WAY with a thin Sharpie:

  1. LABEL TOP OF TUBE: Your FIRST or LAST NAME or YOUR INTIALS
  2. LABEL SIDE OF TUBE: Include: 1) DATE, 2) SAMPLE ID, 3) Your LAST NAME
  3. PLACE SCOTCH TAPE OVER WRITING ON SIDE TUBE

    Place ONE layer of scotch tape OVER the writing on the side of the tube, which helps prevent ink from coming off when the sample is sitting on crushed ice. (Do not use color lab tape.)

SAMPLE SHIPMENT NOTES:

  1. Include a document in the box that has your name/phone number and information about the total number of samples that are in the box and a [short] summary of exactly what type of analysis you are requesting.

  2. Send samples on dry ice.
